fix: refine deployment().name usage in ALZ pattern module

Description

• Refactored deployment name generation to eliminate the use of deployment().name, enhancing performance during redeployments and within deployment stacks.
• Updated module references in README.md and main.bicep to reflect new version numbers for policy and role assignment modules.
