macos13/macos14/macos15: can’t run more than one concurrent instance per host

this is due to the guest definition’s sda (OpenCore.qcow2), which can’t be made readonly or shared and hence can’t be attached to two running guests at the same time.

to fix this, we need to do a per-guest reflink copy in the monitor.
